I had a colleague / friend who had an identical name and was close in age (5 year difference) to someone actually on the list. As a vendor for many orgs and many contacts, he sent an email out to the world to get ahead of rumors and explain it was a different person.

Problem was he accidentally put everyone in the Cc field instead of Bcc. 100s of recipients.
