If he wins, I think how a second term might play out domestically would depend somewhat on who controls the House and Senate. Having said that, he previously appointed several people to "acting" positions, and I am sure we would see even more of that if Democrats control the Senate. Foreign policy is obviously unique since CICs have more power there than they do with executive orders generally.
