Then they review it to see if the QDRO really IS a QDRO after all. Then they approve it. Then it goes to the 401k company. They review it to see if it's legal. Then they have a checklist. Then legal re-reviews it. Then they do the checklist again. Then they create an account. Then they send a letter saying "you've got $$ here are your options". Then you send it back. Then they remove 20% of whatever you don't roll and send to the IRS as income tax. Then they send you your money.
